When this album was finally released, Sire had changed to Warner distribution and the album then sported the K56530 or SRK6057 catalogue number.
The number on this sleeve and in the matrix of the vinyl is for the Phonogram release that never happened.

The proof sleeve was originally tacked together with clear tape, presumably to support the folder but otherwise shows little in the way of wear. This sleeve is also a slightly diffferent layout to the final released version.

The folder is pristine and the vinyl remains in superb condition with few signs of play.

We've never seen one of these before and will be unlikely to again in my lifetime.
